This form is to report behaviors of concern to the Human Resources department at Truckee Meadows Community College including but not limited to: discrimination, unlawful harassment, hate and bias, retaliation, and sexual misconduct.
If you need to report an emergency or are reporting an incident that poses an imminent risk of harm, contact University Police Department or dial 9-1-1. Please know that completing this report does not substitute contacting the police.
Please remember there are resources available to you. To speak with a counselor in confidence regarding an incident, students may contact TMCC Counseling Center; faculty and staff may contact the Employee Assistance Program, ComPsych.
When submitting this report, please include as many details as possible. Only fields with an asterisk(*) must be filled out in order to submit this form.
Upon receipt of this report, the Title IX Coordinator will review the information submitted and take proper steps to investigate, stop, prevent, and/or remedy prohibited conduct in accordance with College policies.
The Human Resources department respects the sensitivity of the information that may be included in this report and will make all reasonable efforts to protect the privacy of those involved, in accordance with applicable state and federal law, while balancing the need to gather information to address the incident and take steps to eliminate prohibited conduct in order to protect the greater College community.
Truckee Meadows Community College prohibits retaliation of any kind against individuals reporting misconduct or any individuals participating in the reporting or investigation process.
