This is your form for educational benefits. Before completing the form, please fully read the form's "Information and Instructions" pages. Also, please read each question carefully before answering it! It is extremely important that you answer questions accurately.

The following clarification of some questions may be helpful:
Question 12 – Ensure you enter both the name and the address of the training institution where you last received VA educational benefits.
Question 13 – Enter the ending date of the term of training for which you received VA educational benefits at the institution stated in Question 12.
Question 15 – Your answer to this question should be the highest level of education you are seeking in your chosen field, whether it is a certificate, associate, baccalaureate, master's, or doctorate level education.
Question 16 – Enter the name of your chosen TMCC program. Following this information, write any higher level of education you will be seeking in your chosen field (whether it is a certificate, associate, baccalaureate, master's or doctorate level education).
After you register for classes and the above instructions are completed, bring your completed request package to the Veterans' Educational Benefits Office, at the admissions and records office at TMCC, 7000 Dandini Blvd., Reno. Include VA Form 22-5495.
Make sure you have also filled out the Course Submittal Form. There is a small amount of additional documentation for you to complete which should only take a few minutes.
