This is your form for educational benefits. Before completing the form, please read fully the form's "Information and Instructions" pages. Also, please read each question carefully before answering it! It is extremely important that you answer questions accurately.

The following clarification of some questions may be helpful:
Question 9 – Ensure you enter both the name and the address of the training institution where you last received VA educational benefits.
Question 10 – Enter the ending date of the term of training for which you received VA educational benefits at the institution stated in question 9.
Question 13 – Your answer to this question should be the highest level of education you are seeking in your chosen field, whether it is a certificate, associate, baccalaureate, master's, or doctorate level education.
Question 14 – Enter the name of your chosen TMCC program. Following this information, write any higher level of education you will be seeking in your chosen field (whether it is a certificate, associate, baccalaureate, master's or doctorate level education).
Questions 17a and 17b – Complete only if you are still on active duty.
Questions 18 through 21 – Need not be answered.
After you register for classes and the above instructions are completed, bring your completed request package to the Veterans' Educational Benefits Office, at the admissions and records office at TMCC, 7000 Dandini Blvd., Reno. Include VA Form 22-1995 and a copy of Form DD214 Member #4.
Make sure you have also filled out the Course Submittal Form. There is a small amount of additional documentation for you to complete which should only take a few minutes.
