New Applicant Chapter 30, Category II Montgomery GI Bill Active Duty Educational Assistance Program

This is your application for educational benefits. Before completing the form, please fully read the form's "Information and Instructions" pages. Also, please read each question carefully before answering it! It is extremely important that you answer questions accurately.

The following clarification of some questions may be helpful:

Question 10 – Do not include any prior use of the military tuition assistance program when you consider your answer to this question.

Question 12c – Enter the name of your chosen TMCC program. Following this information, write any higher level of education you will be seeking in your chosen field, whether it is a certificate, associate, baccalaureate, master's or doctorate level education.

VA Form 21-686c

This is your Declaration of Status of Dependents. You are entitled to additional monetary benefits based on the number of family dependents you claim. The instructions for the completion of this form should be self-explanatory. While the Form indicates you are required to send proof of dependent status for dependents you are claiming, the Department of Veterans Affairs has waived this requirement. The DVA will ask you to forward these documents only if there is a specific need.

After you register for classes and the above instructions are completed, bring your completed request package to the Veterans' Educational Benefits Office, at the admissions and records office at TMCC, 7000 Dandini Blvd., Reno. Include VA Form 22-1990, VA Form 21-686c and a copy of Form DD214 Member #4.

Make sure you have also filled out the Course Submittal Form. There is a small amount of additional documentation for you to complete which should only take a few minutes.
