Academic/Instructional Lab Software Installation Request

Please read all instructions thoroughly.

To request software to be installed for the upcoming semester, please fill out this form. Instructors are responsible for working with software or textbook publishers to obtain proper licensing and installation media. You should be careful to insure that software you're requesting will be available from the publisher by the submission deadline date. If you can't supply exact details, please submit a preliminary request. Written permission from the publisher is required in order to have textbook software installed on academic computers.

Note: For academic computers using the Windows XP Professional operating system, all software submitted on the request form must be able to run using the Windows XP Professional operating system. We will use Microsoft Office 2007 as the office suite for word processing and spreadsheet work, etc. Any software that interfaces with the Microsoft Office suite must be compatible with Microsoft Office 2007. Software Information:
Please be sure to include in your request any helper applications, browser plug-ins, and accessories, such as Java, RealAudio, Shockwave/Flash, QuickTime, etc. that are necessary to support your program if these are not part of the default installation. Either provide the particular software disks or provide directions on where the software can be obtained. Any additional software must be licensed.

Special Requests/Instructions:
If you need special data libraries, resources, or tools installed with your application, please list them below. If there are known incompatibilities or problems you've encountered in the past with the performance of the software, please state that as well.

If the software must be downloaded, the exact URI/link location is needed. A general Web site (i.e. "just go to the company's Web site...") will not be accepted and the requester will be required to resubmit the form with more detailed information.
Important:

By making a software request, your department affirms that it has purchased sufficient rights to as many licenses for the software as you are requesting that we install in support of your courses, and you agree to provide ITO with: (1) a photocopy of the software license and/or purchase order which documents adequate licensing to cover the complete installation required and (2) the original set of installation disks and any accompanying documentation. Requests that do not fulfill these requirements will not be completed.

Instructors will be notified that software is ready for testing by e-mail and in TMCC This Week. Instructors will be notified by e-mail regarding installation requests that are incomplete, lacking license agreements or original installation media. Incomplete requests will not be acted upon until completed.

It is the instructor's responsibility to test the software's functionality. This must be done during the above mentioned time frame. Instructors must test for possible functions that are used during the whole semester. If instructor's find errors or settings they would like to change, we will consider making those changes before the semester starts. If the instructor fails to test the software, we cannot be held responsible for any malfunction during the semester. The software will be copied to all labs and corrections will only be made whenever time and room scheduling allow. Backup copies of installation media will be made and put on file in the ITO department for possible future installations. Originals will be returned to the requester. It is in the instructor's best interest to test the software to ensure a smooth beginning to the new semester.
